New Patient Psychiatric Evaluation with Medication Management with PMHNP-BC
The initial evaluation includes but not limited to collecting personal and family history, medical history, screening tools, discussing medication recommendations if indicated and formulating a recommended treatment plan to achieve a person centered plan of care.

Follow Up Medication Management with PMHNP-BC
Follow up appointments include but are not limited to review of medication efficacy, evaluating response to medication and ongoing pharmacological recommendations
COST: $175.00
Extended follow up with PMHNP-BC for both medication management and therapy services
Extended follow up appointments include but are not limited to review of medication efficacy, evaluating response to medication and ongoing pharmacological recommendations with the additional time to utilize talk therapy, skills education, and other therapeutic interventions.

New Patient Counseling Intake with LMHC
During this initial intake we will begin building the therapeutic relationship, discuss presenting concerns and treatment goals, and gather some personal and family history so that we can begin working effectively towards your goals and wellness.
COST: $225.00
50 Minute Counseling Session with LMHC
During these ongoing therapy sessions we will continue to work towards your treatment goals utilizing talk therapy, skills education, and other therapeutic interventions.
COST: $175.00
Clear Perspectives, LLC is not partnered with any insurance companies and all treatment is considered out of network. This allows providers to be more available to our patients, maintain the highest level of confidentially and not require a formal diagnosis to seek services. If you have, and want to use, out-of-network benefits with your private insurance company, we can provide you with necessary paperwork to file claims for reimbursement (Superbill).
We also recommend using the app Rembursify for ease of submission of benefits at www.reimbursify.com. We cannot guarantee that you will receive any amount of reimbursement for our services due to the wide variety of insurance policies, procedures and plans. Our services are not covered under the Healthy Indiana Plan (HIP 2.0), Medicare or Medicaid.
We do accept Credit Cards & Health Savings Account (HSA) Cards as a methods of payment.
