Routine Follow-Up Medication Management with PMHNP

This type of appointment includes but is not limited to evaluating response to current medication, discussing possible supplemental support and adjusting the ongoing treatment plan as needed.

Computer Based Attention Deficit Hyperactivity Disorder Assessment for Established Patients OR as an add on to New Patient Appt.

This service is for those who do not have a previous ADHD diagnosis and are wanting to explore this for the first time. This will be required in addition to an initial psychiatric evaluation appointment and will include patient work outside of the appointment time. Cost includes the testing fee and follow up appointments to review the results and recommend treatment. Testing can be done in combination with a new patient evaluation by scheduling “New Pt. Eval + ADHD test” on website.

COST: $200.00

Computer Based Attention Deficit Hyperactivity Disorder Assessment ONLY

This service is for those who do not have a previous ADHD diagnosis and are wanting to explore this for the first time. Cost includes the testing fee and a copy of the results. Does not include treatment planning or prescriptions. Please select “Screening for ADHD” when scheduling an appointment.

Extended Medication Management & Therapy with PMHNP

This service includes the same as that of a medication management follow-up plus additional therapy with a nurse practitioner to engage in talk therapy, skills education and general lifestyle modification.

COST: $225.00

Returning Patient Re-evaluation with PMHNP

This allows the potential for additional time for a reevaluation for patients who have not been seen by a provider at the practice within the last 12 months. This would be appropriate for restarting medications after time off or transferring back to the practice for ongoing care.

Clear Perspectives, LLC is not partnered with any insurance companies and all treatment is considered out of network. This allows providers to be more available to our patients, maintain the highest level of confidentiality and not require a formal diagnosis to seek services. If you have, and want to use, out-of-network benefits with your private insurance company, we can provide you with necessary paperwork to file claims for reimbursement (Superbill) for both appointments and ADHD testing. Please ask your provider for more details during your appointment.

We also recommend using the app Rembursify for ease of submission of benefits at www.reimbursify.com. We cannot guarantee that you will receive any amount of reimbursement for our services due to the wide variety of insurance policies, procedures and plans. Our services are not covered under the Healthy Indiana Plan (HIP 2.0), Medicare or Medicaid.

 We do accept Credit Cards & Health Savings Account (HSA) Cards as a methods of payment.

Refills and Maintaining an Active Patient Status: Since we are a speciality practice, we do require regular follow ups to maintain active status which can be a range of 3-6 months intervals based on specific treatment plans. This will be discussed in more detail when meeting with your provider. If it has been greater than 12 months since your last appointment, please see returning patient status above to schedule an extended review session.
